Why Am I Always Tired? 7 Root Causes Doctors Miss

root cause Feb 03, 2026

You're doing everything "right." You get 7-8 hours of sleep. You drink your coffee. You push through your workouts. Yet by 2 PM, you're fighting to keep your eyes open. By evening, you're too exhausted to enjoy time with your family or friends.

Sound familiar?

You've probably visited your doctor, only to hear "your labs look normal" or "maybe your just stressed." Perhaps they suggested antidepressants or told you to sleep more. But deep down, you know something isn't right.

Here's the truth: chronic fatigue isn't normal, and it's not all in your head. Conventional medicine often misses the underlying root cause because standard lab testing simply doesn't look deep enough.

As a Functional Diagnostic Nutrition practitioner, I've helped hundreds of clients uncover the hidden reasons behind their exhaustion. Let me walk you through the seven most common root causes I see - and what you can do about them.

1.  Adrenal Dysfunction (HPA Axis Dysregulation) - Your adrenal glands produce cortisol, your body's main stress hormone. When you're under chronic stress - whether from work, relationships, poor sleep, or even hidden infections - your adrenal system can become dysregulated.

What happens:

  • Morning cortisol should be high (to wake you up) and evening cortisol should be low (to help you sleep)
  • Chronic stress flips this pattern
  • You wake up exhausted and get a "second wind" at night
  • You crave salt and sugar for quick energy

Why doctors miss it: Standard thyroid panels don't test cortisol patterns. Even when they do test cortisol, it's usually just one morning sample, which doesn't show the daily rhythm.

What to do: A comprehensive cortisol test (like the DUTCH test or salivary cortisol panel) measures your cortisol at four different times throughout the day, revealing the full pattern.

2. Thyroid Imbalances Beyond TSH - Your thyroid is your metabolic thermostat. When it's underactive, everything slows down - including your energy production.

What happens:

  • You feel cold all the time
  • Your hair is thinning
  • You can't lose weight no matter what you try
  • Brain fog makes it hard to concentrate
  • You're tired despite sleeping enough

Why doctors miss it: Most doctors only test TSH (thyroid stimulating hormone). But TSH alone doesn't tell the full story.

You also need to know:

  • Free T3 (active thyroid hormone)
  • Free T4 (inactive thyroid hormone)
  • Reverse T3 (the "brake" on thyroid function)
  • Thyroid antibodies (TPO and TG)

Many people have "normal" TSH but low T3, high reverse T3, or autoimmune thyroid issues that conventional labs miss.

What to do: Request a full thyroid panel, not just TSH. Optimal ranges are different from "normal" ranges - this is where functional testing makes a huge difference.

3.  Hidden Gut Infections and Dysbiosis - Your gut produces about 95% of your body's serotonin and plays a massive role in energy production. When your gut microbiome is out of balance or you have hidden infections, your energy plummets.

What happens:

  • Parasites, H. pylori, candida overgrowth, or SIBO steal your nutrients
  • Inflammation from gut imbalances drains your energy
  • Poor nutrient absorption means you can't make cellular energy (ATP)
  • Bloating, irregular bowel movements, and food sensitivities appear

Why doctors miss it: Standard stool tests only look for major pathogens and often miss:

  • Parasites (which require multiple tests to detect)
  • Bacterial overgrowth
  • Low beneficial bacteria
  • Markers of inflammation and malabsorption

What to do: Comprehensive stool testing (like GI-MAP) or similar functional tests) reveal the complete picture of your gut health, including opportunistic infections, inflammation markers, and digestive function.

4.  Chronic Inflammation and Food Sensitivities - Inflammation is like a fire burning inside your body, and it's incredibly energy-draining. Your immune system is constantly "on" fighting what it perceives as threats.

What happens:

  • Joint pain and muscle aches
  • Brain fog and difficulty concentrating
  • Headaches or migraines
  • Skin issues (eczema, acne, rashes)
  • Weight gain or inability to lose weight
  • Exhaustion that worsens after eating certain foods

Why doctors miss it: Doctors may test for celiac disease or severe allergies, but they rarely test for food sensitivities or check comprehensive inflammation markers like hs-CRP.

What to do: An elimination diet (removing common triggers like gluten, dairy, eggs, soy, corn) for 3-4 weeks can be incredibly revealing. Functional testing for food sensitivities and inflammation markers provides additional data.

5.  Nutrient Deficiencies - You can't make energy without the right raw materials. Many people are deficient in key nutrients needed for cellular energy production.

Critical energy nutrients:

  • Iron: Needed to carry oxygen to your cells. Low iron = low energy, even if you're not "anemic"
  • B vitamins: Essential for converting food into ATP (cellular energy)
  • Magnesium: Required for over 300 enzymatic reactions, including energy production
  • Vitamin D: Low levels linked to fatigue and depression
  • CoQ10: Critical for mitochondrial energy production

What happens:

  • You feel tired and weak
  • Exercise feels harder than it should
  • You get dizzy when standing up
  • Your nails are brittle, hair falls out
  • Muscle cramps and twitches

Why doctors miss it: Standard labs often only test for severe deficiency (like anemia), not optimal ranges. Ferritin (iron storage) can be "normal" at 30, but many people don't feel good until it's above 70-100.

What to do: Request comprehensive micronutrient testing including ferritin, vitamin D, B12, folate, and magnesium (RBC) (not serum magnesium, which is inaccurate).

6.  Mitochondrial Dysfunction - Your mitochondria are the powerhouses of your cells - they produce ATP, the energy currency your body runs on. When mitochondria aren't working properly, you literally can't make energy efficiently.

What happens:

  • Profound fatigue that doesn't improve with rest
  • Exercise makes you feel worse (post-exertional malaise)
  • Muscle weakness and pain
  • Brain fog and cognitive issues
  • Often accompanies chronic illness

Why doctors miss it: There's no standard test for mitochondrial function in conventional medicine. It's often assumed to be psychological or dismissed as chronic fatigue syndrome without investigating the cause.

What to do: Support mitochondrial health through:

  • CoQ10 supplementation
  • Reducing toxic exposures (mold, heavy metals, chemicals)
  • Managing stress
  • Eating nutrient-dense foods
  • Functional testing to identify toxins or infections damaging mitochondria

7.  Blood Sugar Dysregulation - When your blood sugar is on a roller coaster, so is your energy. The crashes leave you exhausted, foggy, and craving sugar or caffeine.

What happens:

  • Energy crashes 2-3 hours after meals
  • Hangry, shaky, or lightheaded between meals
  • Cravings for sugar and carbs
  • Difficulty concentrating
  • Waking up at 3 AM (cortisol spike to raise blood sugar)

Why doctors miss it: Fasting glucose and A1C only catch problems once you're pre-diabetic or diabetic. They don't reveal insulin resistance in earlier stages.

What to do: Requesting fasting insulin along with fasting glucose. Insulin rises years before glucose does. A HOMA-IR score (calculated from glucose and insulin) shows insulin resistance early. Continuous glucose monitoring can also reveal patterns your doctor's office won't catch.

What Now? Getting to YOUR Root Cause

If you're  reading this and thinking "that's me!" to multiple sections, you're not alone. These issues often overlap and create a complex web of dysfunction.

The good news? Once you identify your specific root causes through proper functional testing, you can create a targeted protocol to restore your energy naturally.

Here's what I recommend:

  1. Stop accepting "normal" lab results as the final answer. Normal doesn't mean optimal, and many conventional tests miss the root cause entirely.
  2. Consider comprehensive functional testing that looks at your adrenals, thyroid, gut health, nutrients, and metabolic markers all together.
  3. Work with a practitioner who understands functional medicine and can help you interpret results and create a personalized protocol.

You deserve to wake up feeling refreshed, maintain steady energy throughout the day, and have the vitality to enjoy your life. That's not too much to ask - it's how your body is meant to function.
